Job Description:
A company is looking for a Care Navigator with an Alaska nursing
license.
Key Responsibilities:
Complete phone consultations with patients enrolled in care
management and remote patient monitoring programs
Educate patients about their health conditions and empower them to
manage their chronic conditions
Maintain accurate and up-to-date patient records and respond to
remotely transmitted patient data
Qualifications:
Current, valid, and in good standing Multistate/Compact Nursing
License (LPN/LVN)
3+ years of experience in primary care practice, cardiology,
internal medicine, home care, or chronic care management
Reliable internet connection and HIPAA-compliant work area
Proficiency with electronic health records (EHR) and virtual
communication tools
Additional state licenses may be required and reimbursed
