Job Description:
A company is looking for a Proactive Titration LPN.
Key Responsibilities
Contact patients to review medication titration recommendations and
ensure adherence to treatment plans
Maintain detailed documentation of patient interactions in
compliance with HIPAA and clinical protocols
Provide empathetic communication with patients and healthcare
providers while managing responsibilities independently
Required Qualifications
Active LPN license in a Compact Nursing State
5+ years of clinical experience in acute or ambulatory care,
preferably in cardiology or nephrology
Experience maintaining HIPAA-compliant environments
Must have a HIPAA-compliant home office setup with reliable
internet
Strong track record of working independently and managing time
effectively
